The Quality Improvement Department focuses on prevention health and servicing our members by contracting with high quality providers to render services to meet member needs. As a department we work alongside our health plan partners to strategize on key areas of focus based around Healthcare Effectiveness Data Information Set (HEDIS) parameters, guidelines and data. LaSalle serves as a resource for all providers in the network and most importantly for our members to understand how quality drives health equity. The Preventative Care Advocate team conducts outbound calls to educate patients on the importance of preventative care while targeting health disparities in the region.
In order to continue to be a resource for all providers and members, LaSalle continues to develop new methods which will help close out care gaps across the network. Having an active presence in the region and staying at the forefront of HEDIS guidelines is part of LaSalle's goal in obtaining continued recognition for quality efforts. In order to be part of this growing initiative, LaSalle has continued to be innovative in strategy in order to impact member lives across the network.

A full diagnostic review is completed using internal data on provider quality scores, to properly identify areas of focus
Our Preventative Care Advocates will work closely alongside office staff to provide support around HEDIS measure parameters and revision of data
A developed plan of best practices is shared with providers in order to support HEDIS goals
A comprehensive review of all health plan progress reports is conducted to create strategies around focus areas
The preventative care advocates outreach to members via personal and automated text messages in order to maximize on patient engagement
Appointment scheduling support is provided to all members. During these calls we discuss importance of preventative care.
